Birth:  Hep B

2 months: Pediarix (DTaP, IPV, Hep B), Hib , Prevnar, Rotavirus

4 months: same as 2 months

**6 months: Pediarix, Prevnar , (Influenza), Rotavirus

1 year: MMR, Chickenpox, Prevnar (Influenza)

18 months: ***Hib, DTaP, Hep A (Influenza)

2 years:  Hep A (Influenza)

5 years:  MMR, DtaP, IPV, Chickenpox  (Influenza)

11 years:  TdaP, meningococcal, (Influenza)

**(yes, that means one extra does of Hep B, but less needles)

***nationwide shortage, not expected to be fixed for awhile, so probably won't be given
